I think this would look really cool on the back of the neck, where your neck meets your shoulders.
I have 2 and am currently planning my 3rd (a small piece for the back of my neck) and my 4th (a large whole back piece). I love tattoos (well ones that have been done well)!
Regarding pain, I found it was more of burning sensation more than anything and the most painful bit is the outline.
